Add email field to feedback form
Can we add a field to the feedback form that asks for an email/BlazerID? We are getting requests/issues reported, but sometimes we only have the anonymous email attached with the report so the only way to reach back out is through push notifications.

It would be great to be able to customize the feedback form. Such as including instructions and asking for the user's email.

We're struggling with this on our open day profile. People's issues can be resolved quickly, but with no way of reaching out to them via an official support channel.
